As a key member of our team, you'll develop and maintain relationships with physicians, social workers, and discharge planners to ensure a smooth transition from medical facilities to home care. Your expertise in nutrition therapy and customer service will be invaluable in growing our business and expanding relationships with existing referral sources.

Key Responsibilities:

  • Support and assist physicians, social workers, and discharge planners in identifying appropriate home nutrition therapy candidates.
  • Perform hands-on TPN therapy teaching for patients.
  • Facilitate patient discharge planning and coordination of care between Coram and referral sources.
  • Evaluate clinical and service needs for existing and new accounts.
  • Grow business by recognizing opportunities to expand relationships with existing referral sources.

Requirements:

  • Registration with the American Dietetic Association.
  • Minimum one year clinical nutrition support experience.
  • Experience with Microsoft Office, Excel, Outlook, and Word.
