Job Summary

We are seeking a highly skilled and compassionate Community Health Worker to join our team at UnitedHealthcare. As a Community Health Worker, you will play a vital role in supporting our maternity population by providing education, resources, and advocacy to ensure the best possible health outcomes.

Key Responsibilities
  • Assess and develop individualized care plans for maternity patients, taking into account their unique needs and circumstances.
  • Provide education and support to patients and their families on healthy pregnancy and parenting practices.
  • Connect patients with community resources and services to address social determinants of health, such as housing, food access, and transportation.
  • Collaborate with healthcare providers and community organizations to ensure seamless care coordination.
  • Document patient interactions and progress in a timely and accurate manner.
Requirements
  • High school diploma or equivalent required; bachelor's degree in a health-related field or social work preferred.
  • 2+ years of experience working in a community health setting, preferably in a field-based role.
  • Intermediate level of computer proficiency, including MS Word, Excel, Outlook, and multiple web applications.
  • Access to a designated quiet workspace in your home with the ability to secure Protected Health Information (PHI).
  • Must reside in Kentucky within a commutable distance to Hazard, Pikeville, and surrounding communities.
Preferred Qualifications
  • Bachelor's degree in a health-related field or social work.
  • Community Health Worker (CHW) State course completion.
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant.
  • Background in managing populations with complex medical and behavioral needs.
