Job Summary

We are seeking a highly motivated and experienced Care Coordinator to join our team at Spread Your Wings, LLC. As a Care Coordinator, you will be responsible for providing supportive living services to individuals with intellectual disabilities in the Santa Rosa area.

Key Responsibilities
  • Supervise and coordinate the activities of direct care staff to ensure the highest level of care and support for our clients.
  • Train and supervise staff to ensure they have the necessary skills and knowledge to provide quality care.
  • Monitor and report any incidents or concerns related to client care in a timely manner.
  • Manage client finances and ensure that all financial transactions are accurate and up-to-date.
  • Act as a liaison between clients, families, service coordinators, and other agencies to ensure seamless communication and coordination of care.
  • Manage client caseloads and ensure that all client records and charts are up-to-date and accurate.
  • Monitor and evaluate the quality of service delivery and make recommendations for improvement.
  • Coordinate and ensure that all daily activities are completed efficiently and effectively.
  • Maintain current knowledge of Supported Living Services, Federal and State laws, and regulations, including Title 17.
  • Maintain confidentiality and handle sensitive information with discretion.
Requirements
  • Experience in people management and case management, preferably in the disability services sector.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • High level of organization and time management skills.
  • Ability to maintain confidentiality and handle sensitive information.
