Key Responsibilities

Embody the Values, Mission, and Philosophy of Eben Ezer Lutheran Care Center. Assess and monitor residents' medical, psychological, financial, and emotional needs, reporting to appropriate agencies and individuals as required. Complete necessary documentation related to residents' medical, psychological, financial, and emotional status. Oversee the hiring, training, mentoring, and performance evaluation of AL QMAPs. Supervise medication administration by QMAPs at both facilities and provide essential training as needed. Assist residents and families with emotional and psychological needs, particularly in resolving family or resident issues. Support admission screenings, admission processes, and discharge planning. Provide assistance to residents and families with various concerns as required. Ensure accurate and timely documentation and updates to care plans. Attend psychotropic drug review meetings and present necessary information regarding residents. Participate in incident reporting and investigations related to abuse or neglect as needed. Communicate residents' conditions and significant issues with physicians, families, staff, and administration as necessary. Assist in survey preparations and ensure regulatory compliance, supporting the administrator with quality improvement initiatives. Conduct regular rounds to monitor all residents. Administer prescribed medications and treatments in accordance with the Nurse Practice Act, understanding the nursing implications of drug actions, interactions, and adverse effects related to patient diagnoses. Safeguard and administer medications in compliance with legal and policy standards. Take responsibility for personal and professional development. Maintain confidentiality of all work-related information in accordance with HIPAA regulations. Acquire necessary knowledge of State and Federal regulations. Contribute to the development of policies and procedures relevant to job responsibilities. Ensure residents' rights, privacy, and HIPAA regulations are upheld. In addition to the responsibilities outlined above, the role may encompass other duties as assigned based on the needs of Eben Ezer.

Required Qualifications

Essential Qualifications: To successfully perform this role, an individual must meet each essential duty satisfactorily. The qualifications listed below represent the knowledge, skills, and abilities required.

Education and Experience: Must be a Registered Nurse with a current license to practice in Colorado. LPN candidates may be considered. Must possess the ability to supervise QMAPs and demonstrate patience in addressing the needs of residents. Experience in a geriatric healthcare setting is preferred.

Additional Responsibilities: May participate in Eben Ezer committees. May assist with resident transportation on occasion. Attendance at facility and departmental meetings as required.

Skills and Competencies :

Proficient in accurate data entry. Strong critical thinking abilities, attention to detail, and capacity to manage multiple tasks and interruptions throughout the day. Excellent writing and communication skills. Ability to effectively communicate with residents, their families, and other staff members. Exhibit courteous, compassionate, and respectful treatment of residents, families, and colleagues. Strong listening skills and ability to process information effectively. Familiarity with medical terminology. Knowledge of Medicare and Medicaid regulations. Experience in long-term care, working with the elderly, and understanding dementia-related issues.

Equipment and Environment: General office equipment, including phone, fax, copier, adding machine/calculator, and computer. Patient care equipment such as lifts, tubs, wheelchairs, gait belts, beds, and blood pressure cuffs.

Physical Requirements: Must be able to sit, stand, and walk for 8 hours daily. Ability to lift and carry 0-50 pounds regularly and 50+ pounds occasionally with assistance. Must be able to push/pull up to 250 pounds regularly using a wheelchair. Frequent sitting, standing, bending, walking, reaching, lifting, keyboarding, and moving throughout working hours. Exposure to physically ill residents as well as potentially hostile or emotionally distressed residents and families.

Work Schedule: As determined by the Assisted Living Administrator, which may include evenings, nights, weekends, and holidays. Occasional shift work on the floor may be required. Schedule may vary based on operational needs and supervisor discretion.
