Vitas Healthcare is seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses and their families.
Key Responsibilities:- Coordinate care and ensure continuous assessment of patients and families' needs
- Implement the interdisciplinary plan of care and provide education and training to patients, families, and caregivers
- Participate in patient care meetings and provide effective communication and shift reports
- Supervise and delegate nursing tasks to Hospice Aides, Volunteers, and Licensed Practical/Vocational Nurses
- Participate in Quality Assessment and Performance Improvement (QAPI) Program and related activities
- Current licensure to practice nursing in the state where the Vitas program is located
- A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
