About the Role

We are seeking a highly skilled and compassionate Clinical Patient Care Manager to join our team at VITAS Healthcare. As a key member of our interdisciplinary team, you will be responsible for ensuring that patients and families receive continuity and quality of care from admission to discharge or transfer to bereavement.

Key Responsibilities
  • Patient Care Services
    • Direct and oversee all patient care services for the team, including volunteer and bereavement services, to ensure they are rendered as integrated components of the interdisciplinary plan of care.
    • Collaborate with the team physician and attending physician as needed, and act as the clinical resource for the interdisciplinary team.
    • Participate in all potential alive discharges to ensure effective discharge planning.
    • Oversee the team schedule and coordination of services to ensure coverage for all patients on the team.
    • Fulfill patient care responsibilities within their scope of practice during times of need.
  • Staff Supervision and Management
    • Interview, select, train, supervise, evaluate, and dismiss team staff in conjunction with the Patient Care Administrator.
    • Assure staff competence and performance levels through field visits, documentation evaluation, and provision of team-based in-services on VITAS values, hospice principles, and practices.
    • Act as a resource and mentor for staff on clinical issues, documentation, inter- and intra-team problem-solving, and customer service behavior.
    • Oversee staff and volunteer schedules, scheduling, and territory assignments to ensure workload is distributed equitably and that all staff meet overall productivity expectations.
    • Review and approve payroll, ensuring employees have documented accurate time, mileage, and additional expenses.
    • Participate in on-call rotation.
  • Quality Improvement/Regulatory Compliance
    • Monitor clinical records and team activities to ensure they are accurate, complete, and meet required standards in accordance with VITAS policy and all regulatory bodies.
    • Participate in performance improvement activities to respond to service issues and challenges, and documentation issues.
    • Participate in and comply with the QAPI program.
    • Assist as requested by program senior management with surveys and requests from regulatory bodies and intermediaries.
    • Perform required chart reviews to ensure quality care is being delivered.
  • Customer Service/Sales/Marketing
    • Ensure that service issues with patients/families or internal customers are resolved promptly and satisfactorily.
    • Analyze customer service issues on the team to identify causes and work with individual team members and the entire team to improve performance.
    • Actively participate in potential revocations or service concerns.
    • Regularly visit LTC and Contract Bed facilities to ensure care plan integration, customer satisfaction, and to discuss criteria for determining appropriateness.
    • Participate in professional, voluntary, or community service organizations.
  • Cost Containment
    • Verify treatments, staffing, and supplies.
    • Monitor resources and utilization of clinical operations, including labor, pharmacy, medical supplies, equipment, and mileage.
    • Approve all bills/invoices related to patient care services.
    • Oversee staff and volunteer schedules, scheduling, and territory assignments to ensure workload is distributed equitably and that all staff meet overall productivity expectations.
  • Professional Development
    • Attend in-services, educational seminars, and workshops.
    • Develop and achieve professional growth goals and objectives.
    • Participate as a mentor of newly hired Team Managers.
    Requirements
    • Reliable transportation with appropriate license and insurance coverage for driver and passengers.
    • Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care.
    • Basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team, and knowledge of the process of dying and bereavement.
    • Working knowledge of VITAS information system (Vx) and the ability to utilize management reports.
    • Two years of successful supervisory experience or equivalent in a healthcare organization.
    Education
    • Bachelor's degree in Nursing or related field from an accredited college or university or the international equivalent preferred.
    • Current and valid R.N. License to practice in the state where the VITAS program is located.
