Key Responsibilities- Monitor and interpret lab data related to therapeutic drug monitoring.
- Review and assess patients' medication regimens to identify, resolve, and prevent medication-related problems.
- Provide accurate and timely drug information to healthcare professionals.
- Collaborate with healthcare teams to optimize medication therapy and improve patient outcomes.
- Develop and implement systems to promote safe and cost-effective drug therapies.
- Minimum of a Bachelor's Degree in Pharmacy from an ACPE-accredited school.
- Valid RI Pharmacist license.
- Preferred residency or 5 years of clinical experience.
- Board Certification in Pharmacotherapy encouraged.
