Telephonic Nurse Disease Management I

At Elevance Health, we are seeking a skilled and compassionate Telephonic Nurse Disease Management I to join our team. As a Telephonic Nurse Disease Management I, you will play a critical role in delivering patient education and disease management interventions to members across multiple lines of health improvement/management programs for chronic diseases.

Key Responsibilities:
  • Conduct behavioral or clinical assessments to identify individual member knowledge, skills, and behavioral needs.
  • Identify and/or coordinate specific health coaching plan needs to address objectives and goals identified during assessments.
  • Interface with providers and other health professionals to coordinate health coaching plans for the member.
  • Implement and/or coordinate coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.
  • Motivational interviewing is used to facilitate health behavior change.
  • Monitors and evaluates the effectiveness of interventions and/or health coaching plans and modifies as needed.
  • Directs members to facilities, community agencies, and appropriate providers/networks.
  • Refers members to catastrophic case management.
Requirements:
  • Requires AS in nursing and a minimum of 2 years of condition-specific clinical or home health/discharge planning experience; or any combination of education and experience, which would provide an equivalent background.
  • Current unrestricted RN license in applicable state(s) required.
Preferred Skills, Capabilities, and Experiences:
  • BS in nursing preferred.
  • Prior case management experience is preferred.
  • Experience with Coronary Artery Disease and COPD is a plus.
  • Ability to meet necessary internet speed requirements and adhere to work-at-home agreements.
