Advanced Practice Heart Health Professional

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our dynamic Heart Failure team at Trinity Health Of New England's Saint Francis Hospital and Medical Center in Hartford, Connecticut.
In this role, you will have the opportunity to work closely with a multidisciplinary team of healthcare professionals to provide exceptional patient care. Key responsibilities include:
- Managing patients with heart failure, including assessment, diagnosis, and treatment
- Collaborating with cardiologists, hospitalists, and other healthcare professionals to develop and implement patient care plans
- Extending the role of the advanced heart failure physician in managing patients with heart failure, including management of diuretics, guideline-directed medical therapy initiation, and management of cardiogenic shock
The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for providing high-quality patient care. As part of Trinity Health Of New England's commitment to supporting the well-being of its providers, this position offers a competitive base salary of approximately $140,000 - $170,000 per year, depending on experience, along with additional incentives and a comprehensive benefits package.
