IntelyCare is seeking a skilled Registered Nurse (RN) to join our team of healthcare professionals. As a Care Management Nurse, you will be responsible for providing high-quality nursing care to residents in post-acute facilities, such as SNFs and ALFs.

Key Responsibilities:

  • Administer individualized treatments and medications to residents
  • Develop care plans in conjunction with residents and medical teams
  • Set up medical equipment and monitor their use
  • Go over care options with residents and family members
  • Meet and consult with physicians and other healthcare staff as needed
  • Draw blood, urine, and other fluid samples for lab work
  • Maintain accurate documentation of interventions and medical condition histories
  • Participate in investigations into incidents, ensuring accurate reporting
  • Train and supervise nursing staff

Requirements:

  • Graduation from an approved nursing program
  • Post-acute care or in-patient hospital experience (at least 4 months)
