We are seeking a skilled and compassionate Registered Nurse to join our team as a Field RN in New York City.

Key Responsibilities:

  • Collaborate with our multidisciplinary team, including physicians, to assess client needs and develop individualized Plans of Care.
  • Conduct thorough assessments of clients' health care status and psychosocial needs, obtaining physician orders and creating tailored Plans of Treatment.
  • Supervise Home Health Aides, Personal Care Assistants, and Certified Nursing Assistants, providing in-service education and orientation programs as needed.
  • Oversee nurses providing skilled nursing care to infants, children, adolescents, and adults in their homes.
  • Ensure accurate and timely documentation of client care and services.

Requirements:

  • R.N. diploma from an accredited NLN program, with a minimum of an Associate's Degree (BSN preferred). Valid and unencumbered RN license, certificate, or registration, or credentialing as required.
  • At least one year of experience in home health care, with a strong focus on geriatric and pediatric populations.
  • Excellent assessment and organizational skills, with a reliable means of transportation.
