Job Title: Health Wellness LPN

Phaxis is seeking a skilled and compassionate Health Wellness LPN to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality care and support to our clients.

Responsibilities:
  • Conduct thorough needs assessments to identify clients' health and wellness goals.
  • Establish and maintain a network of community resources to facilitate wellness activities and support services.
  • Recommend and lead activity groups, as well as encourage clients' participation in community events.
  • Provide counseling and guidance on wellness-related topics, and monitor clients' progress towards their goals.
  • Collaborate with our interdisciplinary team to ensure comprehensive, client-centered services are delivered and documented.
Requirements:
  • Bachelor's Degree in Human Services, Education, or a related field.
  • Licensed Practical Nurse (LPN) with NYS license.
  • At least two years' experience in benefits and entitlements, with extensive knowledge of community resources.
  • Experience working with HIV/AIDS, chronic mental illness, substance abuse, and homelessness populations is a plus.
  • Bilingual skills in English/Spanish are a plus.
Preferred Qualifications:
  • Knowledge and experience in providing direct services related to treatment and recovery.
  • Excellent oral and written communication skills.
  • Proficiency with MS Office and Google product suites.
