Job Summary:

As a Clinical Care Coordinator RN, you will play a vital role in ensuring the health and well-being of children and youth in state custody. You will work closely with the HOPE Care Manager to coordinate care and integrated services for eligible members, promoting timely access to physical examinations and medically necessary services.

Key Responsibilities:

  • Assess, plan, and implement care strategies that are individualized for children and youth in a family-oriented context.
  • Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services.
  • Manage the care plan throughout the continuum of care as a single point of contact.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for children, youth, and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team.
  • Provide trauma-informed and disease/disorder-specific education to members and caregivers.
  • Coordinate with the child welfare and juvenile justice system on behalf of members and caregivers.
  • Coordinate with additional services, including but not limited to, educational/educational advocacy, complex behavioral health (PRTF, TFC, etc.), services in line with state Family First Prevention and Services Act for children and families, high fidelity wraparound services, early childhood intervention.

Requirements:

  • Current, unrestricted licensure as a Registered Nurse in the state of Michigan
  • 2+ years of child welfare experience
  • 2+ years of working with pediatrics/children
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Reside in the state of Michigan
  • Reliable transportation and the ability to travel within assigned territory to meet with members and providers
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service
