The Practical Nursing Specialist is accountable for delivering exceptional healthcare services to clients at Community Health Net. In collaboration with healthcare providers, this role aims to promote, sustain, and enhance the well-being of patients in a professional and compassionate manner.
This position also involves coordinating referrals to various health and social service agencies, fostering and maintaining collaborative relationships with clients, families, healthcare providers, and colleagues. The Practical Nursing Specialist must be vigilant about the management of medical supplies and equipment, ensuring cost-effectiveness and safety in all practices.
Office Support Responsibilities
In addition to nursing duties, the Practical Nursing Specialist may assist with administrative tasks related to patient registration, discharge processes, medical record maintenance, and scheduling as required.

- Flexibility to work evenings and weekends may be required based on operational needs.
