Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessor. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
Responsibilities- Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
- Work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care as needed
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice, including prescription authority
- Board certification by the NCCPA, AANP, ANCC, or AACN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
