Triage Navigator

We are seeking a Triage Navigator to join our team at Heartland Family Service. As a Triage Navigator, you will be the first point of contact for consumers, completing screenings to determine their needs and matching them with our services and levels of care.

• Complete enrollment and intake for eligible clients
• Develop supportive relationships with clients and respond to their needs during scheduled shifts
• Orient clients to program expectations and address non-compliance as necessary to create a safe and secure environment
• Psycho-educate clients on life, social, and coping skills
• Create and facilitate in-house client life- and social-skill building activities and support groups
• Provide problem-solving and crisis intervention services to empower clients to develop self-sufficiency
• Monitor clients' self-administration of medication and record accordingly
• Provide reports to staff regarding client activities and needs at shift change
• Demonstrate knowledge of current community and HFS programs and resources to meet client needs
• Utilize trauma-informed care, person-centered, and culturally competent principles in providing services
• Complete client room safety checks and searches
• Assist with donation disbursement for successfully discharging clients
• Responsible for cleanliness and upkeep of the facility, including staff office, bathroom, and client common areas
• Place maintenance requests for facility repairs
• Respond to emails, phone calls, and faxes in a professional and timely manner

• High School Diploma
• Previous human service experience desirable, but not required

We offer a competitive compensation package, including a range of $20.42 to $24.70 per hour, based on total years of experience. Our work schedule is 40 hours per week, from 3:00 p.m. to 11:00 p.m., Thursday through Monday, with Tuesdays and Wednesdays off.
