Job Summary:

We are seeking a skilled and compassionate Registered Nurse (RN) to join our dedicated team at Faith Home Health & Hospice. As a Registered Nurse (RN) for Faith, you will provide one-on-one skilled nursing care to patients using a comprehensive, patient-specific care plan set by the Director of Nursing which establishes goals based on nursing diagnosis and incorporates preventative and rehabilitative nursing actions. You will have the support and collaboration of a full team of healthcare professionals.

Key Responsibilities:
  • Provide skilled nursing care to patients in their homes
  • Develop and implement patient-specific care plans
  • Collaborate with healthcare professionals to ensure high-quality patient care
  • Communicate effectively with patients, families, and healthcare teams

Requirements:
  • Current unencumbered, valid Kansas State Board of Nursing RN license
  • Minimum of 1 year of home health experience
  • Valid driver's license, reliable transportation, and auto insurance
  • CPR certification
