We are seeking a skilled Licensed Practical Nurse to join our team as a Patient Care Navigator in Hematology/Oncology. As a key member of our healthcare team, you will play a vital role in guiding patients through their cancer journey, providing education, support, and coordination of care.
Key Responsibilities- Utilize clinical expertise to assess patient needs and develop personalized care plans.
- Collaborate with healthcare providers to ensure seamless coordination of care and effective communication with patients and families.
- Provide education and support to patients and families, addressing their concerns and promoting understanding of their condition.
- Coordinate patient activities, including scheduling appointments, arranging transportation, and facilitating access to community resources.
- Serve as a liaison between patients, families, and healthcare providers, ensuring timely and effective communication.
- Develop and implement patient screening tools and efficient clinic procedures to enhance the patient experience.
- Assist in the development and implementation of patient navigational procedures to improve care coordination and patient outcomes.
- Investigate patient concerns, including appointment access, communication, and billing.
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
- Graduate from a Specialty Training Program in Nursing.
- Minimum of 2 years of experience as a Licensed Practical Nurse.
- Current licensure as a Licensed Practical Nurse in the state of Pennsylvania.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities.
