About the RoleSimpson House, a not-for-profit organization, is seeking a Licensed Practical Nurse (LPN) to join our dynamic team. As a resident advocate and care coordinator, you will play a vital role in providing exceptional care to our seniors.
Key Responsibilities- Develop and maintain relationships with residents and their families, ensuring a consistent caregiver approach.
- Assess and evaluate resident clinical conditions, documenting findings and notifying providers as necessary.
- Update and modify care plans to reflect changes in resident care and service needs.
- Supervise and lead care associates, ensuring high-quality care delivery.
- Administer medications accurately and timely, according to orders.
- Participate in meetings related to resident outcomes and care.
- Respond to resident, family, or responsible party inquiries in a timely and professional manner.
- Participate in clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.
- Graduate of an approved Licensed Practical/Vocational Nursing program.
- A minimum of one year experience in assisted living, home health, or long-term care.
- A passion for caring for seniors and a strong resident advocate.
- Experience with electronic medical records preferred.
- Pa LPN License.
