Position Overview:
The Outpatient Care Specialist is responsible for crisis intervention and stabilization. This role assists clients and their guardians in navigating community resources and completing essential documentation for referrals, admissions, discharges, and out-of-home placements. You will serve as a vital support for the clinical team.

Key responsibilities include performing care coordination functions, ensuring compliance with treatment regulations, and managing authorizations for residential or out-of-home placements.

This position reports to a Program Director.

Work Schedule:


This role operates on a Monday to Friday schedule, with flexibility to work during clinic hours from 8 am to 7 pm.


Compensation:
$20.00 per hour

Key Responsibilities:
- Review and request necessary documentation for members' care levels.
- Assess eligibility for services and benefit packages.
- Develop personalized service plans based on assessments.
- Maintain communication with utilization managers and ensure proper documentation for continued care.

- Coordinate care by collaborating with the consumer's care team to evaluate and adjust treatment plans as necessary.

- Monitor clients' mental health status and treatment responses through direct interactions, updating plans as needed.
- Provide face-to-face crisis intervention and develop safety plans.
- Facilitate transitions to appropriate care levels upon treatment completion.
- Offer information and support to clients and their families regarding community resources.
- Assist clients in scheduling appointments and arranging transportation for necessary services.
- Keep accurate and up-to-date records of client interactions and treatment progress.
- Collaborate closely with health plans, stakeholders, and funding sources for all clients.

Qualifications:

Minimum Requirements:
- Must be at least 21 years of age.
- A combination of education and experience totaling three years is acceptable.
- A Bachelor's degree with one year of full-time experience in behavioral health.
- Valid driver's license and vehicle clearance.

- Must obtain fingerprint clearance from the Arizona Department of Public Safety within eight weeks of hire.
