LICENSED PRACTICAL NURSE - WOUND TEAM

This role involves licensed practical nursing duties that encompass a range of nursing and medical support tasks within a County medical facility. The incumbent will be responsible for delivering direct wound care treatments in partnership with supervising practitioners, as well as executing the facility's wound prevention and treatment initiatives.

NEW WAGE INCREASE

LPN: Annual range = $52,395 - $59,633
Salaries for this position are determined based on verifiable experience.

• Pay rate for candidates with less than five (5) years of experience in the same role: $25.19 per hour

• Pay rate for candidates with at least five (5) years but less than ten (10) years of experience in the same role: $26.88 per hour

• Pay rate for candidates with ten (10) or more years of experience in the same role: $28.67 per hour

Job Description

GENERAL PURPOSE

This position entails licensed practical nursing responsibilities, executing various nursing and medical support functions within a County medical facility. The role is focused on providing direct wound care treatments in collaboration with supervising practitioners and implementing the facility's wound prevention and treatment protocols. Additionally, the position includes dispensing medications, administering treatments, and maintaining accurate documentation in medical records.

SUPERVISION RECEIVED

This role reports directly to a higher-level nursing position.

SUPERVISION EXERCISED

This position oversees the work of Certified Nurse Aides.

ESSENTIAL DUTIES OF THE POSITION

• Collaborates closely with Unit Managers, Charge Nurses, Staff Development Coordinators, practitioners, and Unit Staff to ensure adherence to proper wound prevention principles.

• Implements treatment plans for residents, following established wound care practices.

• Assesses skin conditions using standardized assessment tools and obtains physician orders for treatment modalities as necessary.

• Ensures accurate and timely documentation in medical records, updating reports as needed.

• Conducts weekly rounds, taking photographs and verifying compliance with treatment plans.

• Addresses residents' needs based on recommendations and orders from attending physicians, reporting any significant changes to Registered Nurses (RNs), Nursing Supervisors, or physicians.

• Responsible for timely documentation, assisting with admissions, discharges, transfers, and medication management, as well as notifying RNs, supervisors, physicians, and families as required.

• Administers and accurately records prescribed medications and treatments, monitoring residents' reactions and reporting any adverse symptoms to RNs or Nursing Supervisors.

• Performs routine bedside nursing duties, including but not limited to: bathing, feeding, ambulating patients, taking vital signs, providing skin care, and performing CPR.

• Maintains residents' clinical records through accurate and timely charting of all relevant data and observations.

• Coordinates and leads work responsibilities for Certified Nurse Aides and students when assigned.

• Processes necessary forms and documentation as required.

• Performs additional job duties as necessary.

REQUIRED MINIMUM QUALIFICATIONS

EDUCATION & EXPERIENCE: Graduation from an accredited nursing program; AND

Two (2) years of clinical nursing experience in a healthcare setting, preferably in long-term care, including experience with patient care regulatory documentation; AND

Possession of current licensure as a Licensed Practical Nurse or GPN/Graduate Practical Nurse issued by the Commonwealth of Pennsylvania; AND

Current Basic Life Support (BLS) Certification; AND

Must possess Certified Wound Specialist (CWS), Certified Wound Care Nurse (CWCN), Wound Care Certification (WCC), Wound, Ostomy, and Continence Nurse Accreditation (WOCN/WOC), or Advanced Wound Care Certification upon hire.

OR

Complete advanced wound care training and obtain nationally accredited wound care certification within one (1) year of hire.

GRADUATE PRACTICAL NURSE CANDIDATES: After completing a nursing program, Graduate Practical Nurse candidates who obtain their diploma may be hired if they meet specific requirements.
