Cardiology Medical Office and Heart Failure RN

We are seeking a skilled and compassionate RN to join our Cardiology Medical Office and Heart Failure team in Poughkeepsie, NY. As a Heart Failure Nurse Specialist, you will play a critical role in providing high-quality patient care and education to patients with congestive heart failure.
Key Responsibilities- Assess and implement individualized treatment plans for patients with heart failure, including education on self-management and follow-up care.
- Collaborate with physician leadership to develop quality measures and outcomes reporting processes.
- Manage a panel of high-acuity chronic care patients, primarily those with heart failure, and provide ongoing follow-up contact to ensure compliance with recommendations for self-care.
- Provide telephone advice according to protocol and handle urgent and emergent calls for all patients, including HVCP triage needs per HVCP clinical protocols.
- Responsible for working with the patients' care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan.
- Assists patients in setting goals for self-management tasks and reports abnormal findings to their physician team.
- Collaborates with payer Case Managers for additional services when appropriate.
- Cross-trained to cover HVCP triage as needed.
- Minimum two (3) years' experience in Acute Care nursing or equivalent experience.
- Associates Degree in Nursing required - Bachelor's Degree in Nursing preferred.
- Computer skills/data entry, Microsoft WORD, Excel, Outlook, Presentation skills, Verbal and written English proficiency, etc.
- NYS RN License, ACLS, BLS.
This position requires manual skills/motor coordination and finger dexterity. Some occupational risk is present, and physical effort may be medium to heavy, exerting up to 35 lbs. force. Some exposure to dirt, odors, noise, human waste, etc. is possible.
