Cardiology Medical Office and Heart Failure RN Position

We are seeking a skilled and compassionate Heart Failure Nurse to join our team at Nuvance Health. As a key member of our Cardiology Medical Office, you will play a vital role in providing exceptional patient care and support to individuals with heart failure.
Key Responsibilities- Patient Education and Support: Provide comprehensive education and support to patients with heart failure, including discharge planning, medication management, and lifestyle modifications.
- Clinical Collaboration: Work closely with physicians, nurse practitioners, and other healthcare professionals to ensure seamless care coordination and effective treatment plans.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes, reduce readmissions, and improve overall care.
- Administrative Duties: Maintain accurate and up-to-date patient records, perform data collection and analysis, and assist with administrative tasks as needed.
- Communication and Teamwork: Develop and maintain strong relationships with patients, families, and healthcare teams to ensure effective communication and collaboration.
- Education and Experience: Minimum two (3) years' experience in Acute Care nursing or equivalent experience.
- Certifications and Licenses: NYS RN License, ACLS, BLS.
- Skills and Abilities: Excellent communication and interpersonal skills, ability to work in a fast-paced environment, and proficiency in computer skills and data entry.
This role requires a medium to heavy physical effort, with occasional exposure to dirt, odors, noise, and human waste. The ideal candidate will be able to lift up to 35 pounds and work in a dynamic healthcare environment.
