At Hope Community Support Service, we are seeking a dedicated and compassionate Nurse to join our care team. This role involves providing direct care, monitoring residents' health and wellness, and collaborating with staff to ensure the highest standard of care.

About the Role

The successful candidate will have experience in mental health, residential, or community health settings and be a Registered Nurse (RN) with a valid state license in Maine. They will also possess CPR and First Aid certification and have strong interpersonal and communication skills with a compassionate, resident-centered approach.

Key Responsibilities
  • Resident Care & Health Monitoring:
    • Conduct regular health assessments, respond to residents' medical needs, and oversee prescribed medications administration and treatments.
  • Intervention & Education:
    • Initiate, engage, assess, monitor, educate, coordinate, and provide interventions for residents and their families to improve care quality and health outcomes.
  • Multidisciplinary Collaboration:
    • Participate actively in a multidisciplinary team, attending resident meetings, consultations, internal team meetings, and external stakeholder meetings.
  • Service Planning & Referrals:
    • Assist in developing, implementing, and monitoring Individual Service Plans (ISPs) by participating in resident meetings, outreach to resident and caregivers, collaborating with healthcare providers, and making referrals as necessary.
  • Health Compliance & Program Metrics:
    • Ensure compliance with required health metrics and monitor measures required by the state.
  • Documentation & Data Collection:
    • Maintain accurate and confidential resident records and documentation.
  • Staff Support & Training:
    • Serve as a resource for staff, colleagues, and external members of a resident's care team.
  • Health & Safety Compliance:
    • Follow policies and procedures as outlined by Hope CSS and comply with the Maine State Board of Nursing's nursing scope of practice.
  • Licensure & Driving Requirements:
    • Maintain a valid Maine nursing license, a current driver's license, reliable transportation, and a clean driving record per Hope CSS's policies.
Salary and Benefits

The estimated salary for this position is $65,000-$75,000 per year, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
