About Matrix Medical Network
We offer a range of clinical services that provide primary care providers and at-risk health plan members with the tools and knowledge to better manage their health at home.
As a medical professional, you will work in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Job Summary:
We are seeking a qualified Nurse Practitioner or Physician Assistant to join our team. You will conduct adult/geriatric assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
Responsibilities:
- Conduct assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers and ancillary medical personnel
- Collaborate with Primary Care Physicians on patient education and provide follow-up
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits
Requirements:
- Master's Degree required OR commensurate experience and satisfactory completion of NP licensure
- Current RN and NP or PA licensure in state of practice, including prescription authority
- Board certified by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong computer skills and familiarity with EMR software
- Possess excellent verbal and written communication skills
- Positive and outgoing personality, and be a team player

About the Position:
This is a PRN/part-time position with a flexible schedule. You will have the opportunity to work from home and travel to various locations as needed.
