Overview

We are seeking an experienced Registered Nurse to join our Solid Organ Transplant/Complex Surgical department as a Nurse Care Coordinator. This role offers the opportunity to work with a high-performing team, providing comprehensive care to patients undergoing solid organ transplants and complex surgical procedures.

Salary
The estimated salary for this position is $34,200 per year, based on national averages and industry standards.

Job Description
As a Nurse Care Coordinator, you will be responsible for coordinating patient care, collaborating with interdisciplinary teams, and providing direct patient care services. Your primary focus will be on ensuring seamless transitions between care settings, promoting continuity of care, and improving patient outcomes.

  • • Collaborate with multidisciplinary teams to develop and implement individualized patient plans of care
    • Coordinate patient transfers between departments and facilities
    • Conduct thorough assessments and provide education to patients and families
    • Develop and maintain accurate and comprehensive patient records

Required Skills and Qualifications
To succeed in this role, you must possess strong communication and interpersonal skills, with the ability to work effectively with diverse groups. You should have experience in medical-surgical nursing, preferably in a progressive care unit or transplant setting. Additionally, you should be familiar with electronic medical records systems and possess basic computer skills.
