Job Title: Licensed Practical Nurse (LPN/LVN)

Erickson Senior Living is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team at Eagle's Trace, a beautiful 70-acre continuing care retirement community in West Houston, Texas.

About the Role:
  • Provide clinical care to residents according to nursing standards of practice, board of nursing, and Erickson Senior Living standards.
  • Develop relationships with residents and their families, supporting their physical, spiritual, emotional, and clinical needs.
  • Assess, observe, and evaluate resident clinical conditions, documenting findings in the electronic medical record and notifying providers and RNs as necessary.
  • Update or modify care plans to reflect changes in resident care and service needs.
  • Supervise and lead care associates, administer medications timely and accurately, and participate in meetings related to resident outcomes and care.
  • Respond to residents, family, or responsible parties in a timely and professional manner.
  • Participate in the review of clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.

Requirements:
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • A minimum of one year of experience in assisted living, home health, or long-term care, preferred.
  • A passion to care for seniors and a strong resident advocate.
  • Experience with electronic medical records, preferred.
