Waterfall Community Health Center is seeking a highly skilled and compassionate Mobile Health Unit Coordinator/Patient Navigator to join our team. This role plays a vital part in breaking down social health barriers for patients, ensuring equitable access to care.

As a Mobile Health Unit Coordinator/Patient Navigator, you will be responsible for scheduling the mobile health unit for Waterfall Community Health Center within Coos County and surrounding areas. This will require extensive networking with community partners and leaders to schedule and host dental and medical visits, cancer screening and prevention, vaccine, and other outreach events.

Your duties will include:

  1. Adhering to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties.
  2. Demonstrating the core values of Integrity, Respect, Accountability, Compassion, Partnership, and Collaborative Communication.
  3. Researching and implementing best practices to provide mobile health unit (MHU) quality patient care and serving as the communication link between MHU patients, clinical staff, and the integrated care program; providing outreach to assist patients in navigating the healthcare system and accessing health services and resources.
  4. Routinely documenting all patient interactions in the Electronic Health Record (EHR).
  5. Facilitating data collection, aggregation, and display for all MHU activities.
  6. Attending monthly Quality Improvement (QI) Committee meetings and reporting data on MHU initiatives in collaboration with the rest of the Quality team.
  7. Working in collaboration with the Population Health Coordinator and Clinic Manager to identify patient populations that have difficulty accessing care and creating a plan to provide care to these individuals.
  8. Facilitating interdepartmental collaborations to develop, implement, and carry out MHU events to improve chronic disease management, patient screenings, and closing care gaps for identified patients.
  9. Actively engaging with community partners and leaders from surrounding areas to build and maintain rapport and a collaborative working relationship with WCHC.
  10. Assisting a panel of clients to gain access to health-related social services (including identifying socio-economic issues) that affect patient overall health and wellbeing; developing health/social management plans and goals with patients and acting as a case manager for patients and their health and wellness team.
  11. Continuously expanding knowledge and understanding of community resources/services; facilitating client access to, and assisting clients in utilizing those services, including scheduling appointments with social services agencies and assisting with completion of applications and making phone calls for programs for which they may be eligible.
  12. Assisting patients with eligibility for Medicaid/Oregon Health Plan (OHP) and working to increase OHP members particularly in local marginalized and underserved community populations.
  13. Accompanying the MHU to all events and activities and acting as host for all participants, including Waterfall staff, patients, and community partners.

This role requires a Bachelor's Degree, proficiency in Microsoft Office, and experience with Electronic Health Record systems. The ideal candidate will possess strong organizational, communication, and interpersonal skills to support multiple priorities at multiple locations.

We offer a competitive salary range of $25.56-$28.17 per hour, depending on experience.
