Job Description:We are seeking a highly skilled and compassionate Telephonic Case Manager to join our team. As a Telephonic Case Manager, you will work closely with injured workers, healthcare providers, and employers to develop and implement effective treatment plans that promote recovery and return to work.
Key Responsibilities:
- Conduct thorough assessments of injured workers to identify treatment needs and goals.
- Develop and implement individualized treatment plans that address the unique needs of each injured worker.
- Collaborate with healthcare providers and employers to ensure seamless communication and coordination of care.
- Monitor and evaluate the effectiveness of treatment plans and make adjustments as needed.
- Provide education and support to injured workers and their families to promote recovery and return to work.
Requirements:
- Active RN license in good standing.
- 2+ years of clinical experience in workers' compensation or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic medical records and case management software.
