Key Responsibilities:


The MDS Nurse, also known as the Care Management Nurse, is responsible for overseeing the RAI process, conducting assessments, and coordinating care plans for assigned residents.


Key duties of the MDS Nurse include:
Completing the MDS, CAAs, and care plans within required timelines.
Conducting resident assessments through physical evaluations, interviews, and reviewing medical records.
Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activities personnel to discuss resident care needs.

Gathering and reviewing information from hospitals, consultants, and external agencies to inform assessment and care planning.

Identifying and coordinating significant changes, preparing for PPS meetings, maintaining MDS assessments in accordance with Medicare schedules, and monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator. Staying updated with the requirements set forth by the American Association of Nursing Assessment Coordinators (AANAC).


Qualifications:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) credentials are required. AANC certification is a plus, along with RAC-CT certification. Familiarity with the Resident Assessment Instrument (RAI) process and the principles of the Prospective Payment System (PPS) is highly preferred.
Experience in the role of an MDS Nurse is essential.
