We are seeking an exceptional Ambulatory Care Nurse Specialist to join our team at Texas Children's Hospital. In this role, you will have the opportunity to work with patients and families, providing comprehensive care in a dynamic environment.

Job Summary

As an Ambulatory Care Nurse Specialist, you will be responsible for assessing patient needs, developing individualized plans of care, and implementing evidence-based interventions. You will also collaborate with interdisciplinary teams to ensure seamless transitions of care.

Key Responsibilities:

Evaluate patients' health status, identifying areas for improvement and developing strategies to enhance their well-being.">* Develop and implement individualized care plans, prioritizing interventions and collaborating with patients and families to achieve optimal outcomes.">* Coordinate with healthcare teams to ensure continuity of care, streamlining communication and fostering effective partnerships.">* Provide education and support to patients and families, empowering them to manage their health effectively.">* Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that promote excellence in care.Requirements

To be successful in this role, you must possess a Bachelor's Degree in nursing or an Associate's Degree with a signed agreement for a BSN program. You should also hold a current RN license in the State of Texas and Basic Life Support certification from the American Heart Association. A minimum of one year of professional nursing experience is required.

We offer a competitive salary range of $78,000 - $115,000 per annum, based on experience, as well as a comprehensive benefits package.
