Job Title: Hospice Care Specialist
About the JobWe are seeking an experienced Nurse Practitioner to join our team at Elite Hospice of Hot Springs, a part of LHC Group. As a Hospice Care Specialist, you will provide consultation in palliative care, symptom management, and supportive care to meet the needs of our patients.
Responsibilities- Provide face-to-face encounter visits within prescribed time limits to qualifying hospice patients
- Report clinical findings to the Hospice Medical Director
- Consult with the Hospice Medical Director as needed, inform primary physician of recommended services, and collaborate with other physicians as needed
- Initiate and coordinate the plan of care
- Document problems, appropriate goals, interventions, and patient/family response to hospice care
To be successful in this role, you should have:
- A minimum of five years nursing experience, preferably in oncology, hospice, geriatrics, or intensive care
- Certification as a Clinical Nurse Specialist or Nurse Practitioner in your state
- Current CPR certification
