Hospice RN/RN Case Manager Job Summary

We are seeking an experienced Hospice RN/RN Case Manager to join our team. As a key member of our interdisciplinary care team, you will be responsible for providing direct patient care, collaborating with physicians and other healthcare professionals, and advocating for patients' needs.

  • Patient Care: Provide skilled nursing care to patients in their homes, hospitals, or other settings, as needed.
  • Case Management: Assess patient needs, develop and implement care plans, and coordinate with other healthcare providers to ensure seamless care delivery.
  • Collaboration: Work closely with multidisciplinary teams to share knowledge, expertise, and best practices in patient care.
Requirements
  • Education: Associate's degree in Nursing (ASN) or Bachelor's degree in Nursing (BSN).
  • Licensure: Current Registered Nurse (RN) license in the state of employment or eligibility to obtain.
  • Experience: At least one year of experience in hospice, home health, ICU, geriatric, or oncology nursing.
