We are seeking a highly skilled and compassionate Ambulatory Nurse Specialist to join our team at Thomas Jefferson University. As a key member of our primary care clinical team, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Provide patient-centered care, utilizing the nursing process to address the physical, psychological, emotional, and social needs of patients.
- Collaborate with primary care providers and support staff to ensure seamless care coordination and effective communication.
- Develop and implement evidence-based ambulatory care nursing interventions to promote optimal patient outcomes.
- Provide education and support to patients and their families on preventive care, chronic disease management, and medication management.
- Document patient interactions and clinical care in the electronic medical record, adhering to organizational standards and regulations.
- Perform care coordination tasks, including abnormal result management, triage, and RN verification of vaccinations and other injections.
- Oversee clinical care essential to patient assessment, promotion, maintenance, and restoration of health and wellness.
- Minimize risk of harm to patients by following established nursing practices and protocols.
- Utilize information technology to communicate, manage knowledge, and mitigate errors.
- Use data to monitor care processes and support ethical decision-making, contributing to continuous quality improvement.
- Function effectively within inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
- Graduate of an accredited school of nursing, with a BSN or completion of BSN within 3 years of hire strongly preferred.
- 2+ years' experience as a Nurse in a primary care or ambulatory setting, preferred.
- Current RN licensure in the state of employment and AHA BLS certification required.
