At Molina Healthcare, we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in ensuring that our members receive high-quality, patient-centered care.
Key Responsibilities:
- Conduct comprehensive assessments of members to identify their needs and develop personalized care plans.
- Collaborate with healthcare professionals, caregivers, and members to ensure seamless care coordination and effective service delivery.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Promote integration of services, including behavioral health care and long-term services and supports, to enhance the continuity of care for Molina members.
- Assess medical necessity and authorize waiver services, ensuring that members receive the care they need in a timely and efficient manner.
Requirements:
- Bachelor's or master's degree in a social science, psychology, gerontology, public health, or social work.
- At least 1 year of experience working with persons with disabilities/chronic conditions and Long Term Services & Supports.
- 1-3 years of experience in case management, disease management, managed care, or medical or behavioral health settings.
Preferred Qualifications:
- 3-5 years of experience in case management, disease management, managed care, or medical or behavioral health settings.
- 1 year of experience working with populations who receive waiver services.
