Job Summary

We are seeking a highly motivated and organized Case Manager/Monitor to join our team at ATTIC Correctional Services, Inc. in Green Bay, WI. The successful candidate will provide community-based services to correctional clients following their release and transition back to their home community.

Key Responsibilities
  • Perform client pre-admission, intake, and assessment procedures, including orientation to the program.
  • Distribute food staples, linens, bedding, and other essential items.
  • Shop and purchase necessary items for client intake using an agency credit card.
  • Complete and distribute Incident Report forms as required.
  • Process all receipts and send them to the Admin office with allocation sheets.
  • Maintain petty cash and monthly reports.
  • Maintain client files and notes.
  • Complete client discharge summaries and distribute them to the assigned agent.
  • Maintain current client status in the DOC PDC data system.
  • Ensure coordination of all THS services, including maintaining staff schedules.
  • Complete monitoring checks during scheduled hours.
  • Log entries for each monitoring check, both hard copy and computer.
  • Communicate with coworkers and coordinate monitoring checks.
  • Monitor clients to ensure they are following program policies, procedures, and rules.
  • Monitor the physical plant to ensure the health, safety, and security of clients, the community, and property.
  • Respond to emergency issues and alert DOC, law enforcement, coworkers, and the ACS Supervisor.
  • Communicate client issues with the assigned agent.
  • Ensure apartments are maintained in a clean and sanitary condition at all times.
  • Communicate with landlords/maintenance as needed.
  • Keep lines of communication open with the Supervisor and respond to any inquiries made.
  • Adhere to all rules and regulations.
Qualifications
  • Access to a personal vehicle.
  • A valid driver's license, good driving record, and proof of insurance.
  • Ability to travel frequently.
  • Demonstrated understanding and sensitivity to interactions with individuals from diverse backgrounds and cultures.
  • Excellent verbal and written communication skills.
  • Ability to interact with staff, clients, and other parties in an effective manner while maintaining professional boundaries.
  • Strong decision-making abilities and self-motivation.
  • Ability to maintain the utmost confidentiality of all agency information.
Education and Work Experience
  • High School diploma or equivalent.
  • Previous experience working with correctional clients is preferred.
