Job Summary:

The successful candidate will have a strong background in nursing, with a focus on ambulatory care and occupational health. You will be responsible for providing direct and telephone nursing care, health teaching, patient advocacy, and coordination of care given by others. Your expertise will be essential in ensuring that patients receive timely and effective care, while also promoting a safe and healthy work environment.

Responsibilities:
  • Provide direct and telephone nursing care to patients, including health teaching and patient advocacy
  • Coordinate care given by others, including supervision and coordination of services required for patients outside the ambulatory care setting
  • Develop and continuously update individualized plans for patients seeking care for health promotion, health malignance, or health-related situational problems
  • Prioritize interventions and implement the plan of care in partnership with the patient, family, caregivers, and other healthcare professionals
  • Conduct reevaluation and follow-up of care delivered based on the patient's needs, condition, or request
  • Use ongoing reassessment data to revise diagnoses, expected outcomes, and implementation of the plan as needed
  • Demonstrate patient advocacy as evidenced in response to patient care issues and collaboration with the healthcare team
Requirements:
  • Current licensure with the Board of Nursing for the State of Texas or Nurse Licensure Compact
  • 1 year of professional nursing experience
  • EPIC experience highly preferred
