Healthcare Triage Coordinator

We are seeking a highly skilled Healthcare Triage Coordinator to join our team at Shasta Community Health Center. In this role, you will be responsible for providing independent telephone or face-to-face triage using written protocols and performing medication refills according to nurse refill protocol.

As a Healthcare Triage Coordinator, you will also utilize the nursing process (assessment, planning, intervention, and evaluation) in patient care and daily activities; documenting all actions, as appropriate, in EHR. You will have the opportunity to participate in the quality improvement process as requested or assigned and provide direct patient education.

Additionally, you will accept and correctly transcribe verbal or telephone orders from clinicians and call in new prescriptions as well as prescriptions with changes in dose, duration, method of administration or frequency.
