About the Role

The Ambulatory Triage Registered Nurse is a vital member of the Anderson Family Medicine Care team. This role involves conducting duties such as phone triage, patient education, scheduling appointments, referrals, prescription refills, care coordination and other related tasks.

Responsibilities

This position requires:

  • ">
  • Conducting phone triage to assess patient needs and provide guidance on next steps.">
  • Providing patient education on various health topics and promoting healthy behaviors.">
  • Scheduling appointments, referrals, and prescription refills in a timely manner.">
  • Care coordinating with patients, families, and healthcare teams to ensure seamless care delivery.">
  • Performing other related tasks as needed to support the efficient operation of the office.">

Requirements

To be successful in this role, you must possess:

">
  • ">
  • A graduate degree from an accredited school of nursing (NLN, CCNE, ACEN, or NLN CNEA).">
  • A valid license to practice as a Registered Nurse (RN) in the state of Indiana, as listed in the Nurse Licensure Compact (NLC).">
  • At least three years of experience in nursing, preferably in a medical office or outpatient setting.">
  • Excellent communication and team skills, with the ability to work effectively in a fast-paced environment.">
  • A compassionate and caring attitude, with a strong commitment to providing exceptional patient care.">
  • Accountability and self-direction, with the ability to work independently and as part of a team.">
  • EPIC experience is a plus.">
