About the Position

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time provider. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide treatment plans for patients in their homes and skilled facilities.

Responsibilities
  • Conduct adult and geriatric assessments, medical history reviews, and provide treatment plans for patients in their homes and skilled facilities.
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care.
  • Provide education and support to patients and their families to promote healthy behaviors and disease management.
  • Collaborate with primary care physicians to ensure seamless care coordination.
  • Conduct telehealth visits and provide remote care to patients who require it.
Requirements
  • Master's degree in nursing or a related field.
  • Current RN and NP or PA licensure in the state of practice.
  • Board certification in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
