Enrollment Nurse

WelbeHealth is seeking a skilled and compassionate Enrollment RN to join our team. As a key member of our outreach and enrollment team, you will play a vital role in guiding the eligibility process and helping seniors gain access to our program.
Job SummaryThe Enrollment RN will conduct initial nursing assessments, including nursing evaluations, functional assessments, and home safety evaluations, to determine if prospective participants meet PACE eligibility requirements. You will also prepare documentation from home visits, submit it to the CA State Level-of-Care (LOC) nurse for review, and respond to questions to expedite the review process.
Key Responsibilities- Conduct initial nursing assessments to determine PACE eligibility
- Prepare and submit documentation from home visits
- Track and oversee the annual State level-of-care review process
- Collaborate with the interdisciplinary team and outreach & enrollment team
- Protect privacy and maintain confidentiality of company procedures and information
- Graduate of an Accredited School of Nursing required; B.S. Nursing preferred
- Unencumbered RN License in state with which you practice required
- Minimum of two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
- Reliable means of transportation
- At least 1 year of Home Health experience Preferred
- Spanish Required

Salary/Wage base range for this role is $57.29 hourly + Bonus + Equity. WelbeHealth offers a competitive total rewards package that includes 401k match, healthcare coverage, and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
