Job Summary

We are seeking a skilled Enrollment RN to join our team at WelbeHealth. As a key member of our outreach and enrollment team, you will play a vital role in guiding the eligibility process and helping seniors gain access to our program.

Key Responsibilities
  • Conduct initial nursing assessments, including nursing evaluation, functional assessment, and home safety evaluation, to determine if prospective participants meet PACE eligibility requirements.
  • Promptly prepare documentation from home visits and submit it to the CA State Level-of-Care (LOC) nurse for review and reply to questions to expedite the review process.
  • Track and oversee the annual State level-of-care review process.
  • Collaborate with the interdisciplinary team (IDT) and outreach & enrollment (O&E) team.
  • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
Requirements
  • Graduate of an Accredited School of Nursing required; B.S. Nursing preferred.
  • Unencumbered RN License in state with which you practice required.
  • Minimum of two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred.
  • Reliable means of transportation.
  • At least 1 year of Home Health experience Preferred.
  • Spanish Required.

Salary/Wage base range for this role is $57.29 hourly + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
