  • Must demonstrate leadership and supervisory capabilities and be willing to work cooperatively with and oversee other staff members.
  • Must have the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures necessary for delivering quality care.
  • Must possess patience, tact, a positive attitude, and enthusiasm, along with the willingness to manage challenging residents.
  • Must be open to exploring new methods and principles and integrating them into existing nursing practices.
  • Must be able to communicate information regarding a resident's condition effectively.
  • Must not pose a direct threat to the health or safety of others in the workplace.

Key Responsibilities:
Responsibilities encompass but are not limited to the following: Care Plan Assessment & Administrative Duties

  • Oversee and coordinate the formulation and completion of resident assessments, Minimum Data Set (MDS), and Care Planning in accordance with current regulations and guidelines governing resident assessments in Avante facilities.
  • Ensure timely completion and submission of all MDS Assessments to the State repository as per the RAI Manual regulations.
  • Confirm that all residents meet skilled care coverage criteria as per governmental or other regulatory standards.
  • Facilitate collaboration among the Interdisciplinary Care Team in each facility to select the most suitable Assessment Review Date.
  • Coordinate the review and modification of the resident's care plan by the interdisciplinary team following each quarterly review or other assessments, ensuring that the care plan is evaluated and updated with each assessment or change in the resident's condition.
  • Monitor Quality Measures Report monthly, focusing on measures affecting the Federal Five Star System to ensure accurate coding of residents in accordance with RAI guidelines.
  • Assist nursing staff in encouraging resident and family participation in the development and review of the resident's care plan.
  • Ensure that all personnel involved in resident care are informed of the resident's care plan and that nursing staff refer to it prior to administering daily care.
  • Participate in facility inspections conducted by authorized government agencies.
  • Execute administrative tasks such as completing medical forms, reports, evaluations, studies, and charting as necessary.
  • Commit to confidentiality regarding assigned user ID codes and passwords for accessing resident/facility information and promptly report any suspected or known breaches to the Administrator.
  • Maintain confidentiality of resident's protected health information and report any suspected or known violations to the Administrator.
  • Report any known or suspected unauthorized attempts to access the facility's information system. Personnel Functions & Staff Development
  • Engage in the facility orientation process.
  • Consult with department supervisors regarding resident assessment/care plan functions.
  • Develop and participate in planning, conducting, and scheduling timely in-service training sessions that include assessment skills or techniques necessary for completing assessment functions.
  • Attend annual facility in-service training programs as scheduled.
  • Report known or suspected incidents of fraud to the Administrator. Nursing Care Responsibilities
  • Conduct or coordinate interviews with each resident for their assessment.
  • Ensure fair treatment of all residents, with kindness, dignity, and respect.
  • Evaluate each resident's condition and relevant medical data to identify any need for special assessment activities or amendments to the admission assessment.
  • Assist facility directors and supervisors in scheduling resident assessment/care plan meetings.
  • Facilitate participation by external members of the care plan team, including the resident's representative and/or other interested family members.
  • Ensure that care provided aligns with the resident's preferences.
  • Safeguard the confidentiality of all resident care information, including protected health information, and report any known or suspected unauthorized disclosures.
  • Ensure resident interviews are conducted privately.
  • Ensure all members of the assessment team and interdisciplinary care planning/medical and nursing team are knowledgeable of residents' rights and responsibilities, including the right to refuse treatment.
  • Ensure nursing staff respect residents' treatment refusal requests, in accordance with facility policies on advance directives.

Work Environment:

  • Operates in office areas as well as throughout the nursing service areas (i.e., nurses' stations, resident rooms, etc.).
  • Moves intermittently during working hours and is subject to frequent interruptions.
  • Engages with residents, family members, staff, visitors, and regulatory agencies under various conditions and circumstances.
  • May encounter hostile and emotionally distressed residents, family members, staff, and visitors.
  • Works beyond standard hours, including weekends and holidays when necessary.
  • May be required to assist in resident evacuation during emergencies.
  • May be exposed to risks such as falls, burns from equipment, odors, and reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • May be subject to exposure to infectious waste, diseases, and conditions, including TB and the AIDS and Hepatitis B viruses.
  • May handle and be exposed to hazardous chemicals.
  • Maintains communication with residents, their families, support departments, etc., to effectively plan for residents' needs.

Physical Job Requirements:
(With or Without the Aid of Mechanical Devices) While performing the duties of this role, the employee is occasionally required to stand; walk; sit; use hands to manipulate objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch, or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 20-50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Education and Experience:

  • Must possess, at a minimum, a Nursing Degree/Diploma from an accredited nursing school, college, or university.
  • Preferred, at least two (2) years of experience in a hospital, nursing care facility, or other related healthcare setting.
