We are seeking a dedicated and compassionate RN Case Manager to join our Home Health team. As a key member of our care team, you will provide personalized care and support to patients in the comfort of their own homes.
Responsibilities:
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
- Provide hands-on care and education to patients and their families, promoting independence and self-management.
- Monitor patient progress, adjust care plans as needed, and ensure timely interventions to prevent complications.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
- Diploma, Associate, or Bachelor Degree in Nursing
- A minimum of one year of nursing experience, preferably in home health or a related setting
- Strong med surg, ICU, ER, acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation
