Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide skilled nursing visits, case management, and follow-up care to patients in their homes.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Implement and evaluate patient care plans, making adjustments as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and continuity of care.

Requirements:

  • Diploma, Associate, or Bachelor Degree in Nursing
  • A minimum of one year of nursing experience, preferably in home health or a related field
  • Strong med surg, ICU, ER, acute experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
