Job Summary:The Private Duty Nurse LPN will be responsible for implementing physician-prescribed plans of care, conducting patient assessments, and coordinating care with other healthcare professionals.
Responsibilities:- Implement physician-prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in a timely and compliant manner
- Teach and train family members on health promotion and disease prevention
- Accept and write verbal orders from physicians as required, and obtain cosignature of Case Manager on all verbal orders
- Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and changes in client condition
- Active, unencumbered LPN license in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Six months prior handson nursing experience preferred but not required
- Reliable transportation
