Job Title: Private Duty Nurse LPN

We are seeking a compassionate and skilled Private Duty Nurse LPN to provide high-quality care to our patients in their homes.

Job Summary:

The Private Duty Nurse LPN will work closely with our patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:
  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease prevention
  • Accept and write verbal orders from physicians as required
  • Follow up with physicians, clients, family members, and staff on client problems and concerns
Requirements:
  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred
  • Reliable transportation
