As a Client Care Coordinator at Hackensack Meridian Health, you will play a vital role in ensuring the seamless delivery of home care services to our patients. This is an exciting opportunity to join a dynamic team that is dedicated to transforming healthcare and serving as a leader of positive change.
Key Responsibilities- Manage client and caregiver scheduling, tracking visits, and case conferences to ensure efficient and effective service delivery.
- Schedule visits following standardized processes, verifying physician orders and updating client schedules on a daily basis to reflect current needs.
- Verify authorizations, notify clinical supervisors of activities requiring direction or input, and track disaster plans as necessary.
- Validate caregiver documentation against schedules, reconcile discrepancies, and notify supervisors of outstanding documentation.
- Recognize patient grievances, direct them to the appropriate manager, and assist the on-call RN with weekend and daily on-call processes.
- Ensure accurate and timely scheduling of weekend visits and SOC entries into the system.
- Maintain client and employee schedules accurately, enter information into the system in a timely manner, and fax transfer forms to facilities and coordinators within 24 hours.
- High School diploma, GED, or equivalent; Associate's Degree preferred.
- Two years of customer service experience.
- Proficient in Microsoft Office Suite.
- Strong organizational skills.
- Healthcare experience preferred.
