About the Role

We are seeking a skilled Life Skills Clinician to join our Behavioral Health team at Community Health Network. As a Life Skills Clinician, you will play a critical role in providing care coordination and life skills training to individuals with mental health and/or substance use disorders.

Key Responsibilities
  • Collaborate with a multidisciplinary care team to develop and implement comprehensive treatment plans.
  • Provide life skills training and coaching to individuals with mental health and/or substance use disorders.
  • Conduct comprehensive psychosocial assessments and gather data to inform treatment plans.
  • Document services provided in Electronic Medical Records (EMR) based on policies and procedures.
  • Transport clients in personal vehicles when necessary.
Requirements
  • Bachelor's Degree in mental health or a related field.
  • Current Indiana Driver's License.
  • Two (2) years of social service experience in a volunteer or paid role (preferred).
