We are seeking a highly skilled Rehabilitation Care Professional to join our team at Adoration Home Health and Hospice. This is a unique opportunity to work in a dynamic and supportive environment, utilizing your expertise to make a positive impact on the lives of our patients and their families.

The estimated salary for this position is $90,000 - $120,000 per year, depending on experience and qualifications. Our comprehensive benefits package includes medical, dental, and vision insurance, as well as opportunities for professional growth and development.

Job Description:

  • Develop and implement personalized treatment plans to address patients' daily living and work-related skills, and monitor progress towards goals.
  • Collaborate with interdisciplinary teams to design and implement community reintegration programs that promote patients' physical and cognitive abilities.
  • Assess patients' needs and provide recommendations for adaptive equipment solutions to improve function in home, community, and work environments.
  • Train patients and staff on various skills, including dressing, feeding, grooming, hygiene, and community re-entry, and evaluate training effectiveness.
  • Promote effective communication and collaboration among patients, families, and healthcare professionals to achieve individualized goals and outcomes.

Required Skills and Qualifications:

  • Bachelor's degree in Occupational Therapy.
  • Maintain professional license and/or certification as required by the state.
  • 1 year of experience preferred.
  • Demonstrated knowledge of rehabilitation techniques related to complex neurological injury.
  • Excellent communication and interpersonal skills.
